The lack of adequate communication between patients and health professionals may hamper patient-centred care and shared decision-making. This requires attention to promoting understandable two-way health dialogue and efforts by health professionals and health authorities to find better ways to reach all people.

Myths About Living Wills: Providing Expertise to Reduce Misconceptions When Advising on Advance Directives (MyPatH) - Germany

 Aim and objectives:

Misconceptions about advance care planning (ACP) documents, such as living wills and durable powers of attorney, prevent these tools from reaching their full potential. A significant lack of information persists among both counseling healthcare professionals and the general public. The overarching goal of the MyPatH project is to improve the provision of target-group-specific information on ACP documents to enhance overall health literacy.

 Target group:

General population

 Method:

The MyPatH project first identifies existing knowledge gaps regarding ACP documents among healthcare professionals across various disciplines and the general public. Building on these findings, needs-based information concepts and continuing education modules were developed for both target groups and evaluated regarding user acceptance and knowledge gain. In addition to low-barrier, small-group training sessions, the project utilizes e-learning formats designed for self-directed learning.

 Outcome and added value:

Based on the identified knowledge gaps, the MyPatH project establishes measures to increase health literacy among the general public and healthcare professionals while countering common misconceptions. Initial information formats developed during the project will undergo continuous testing, optimization, and practical implementation. In the long term, these efforts aim to maximize the practical benefits of advance care planning documents.
